Identification card | The patient's original NRIC |
Immediate family members' particulars | Fill in the information on the patient's immediate family members as listed in the attached form (Part I).

Proof of income (patient and all immediate family members) | Provide any one of the following documents for the patient and each of the immediate family members:

Proof of savings | All of the patient's and spouse's updated bankbooks or bank statements (including personal, joint, fixed deposit). |
Latest CPF statement (patient and spouse) | Provide the past 12 months CPF transaction history for patient and spouse including the Ordinary, Special, MediSave and Retirement account balance. |
Patient's insurance policies | The patient's insurance policies such as medical, hospitalisation, critical illness, accident, endowment, savings or life plans. |
Breakdown of household expenses | Fill in the patient's monthly household expenses as listed in the attached form (Part II). |
Household bills | These can include rental, utilities, condo maintenance fees, or telephone bills. |
Proof of loan and installments | Documentary proof for any loans or installments. |
(If applicable) Any other supporting documents | Provide any other documents that reflects your current financial resources (e.g. rental income, contribution from family or friend) and commitment. |
*Note: Applications with incomplete documents for more than 1 month from the date of appointment with your medical social worker will be considered withdrawn.
