Nurse Clinician Mas Rizalynda ensures patients receive the best possible care, not just by treating their diseases or conditions but by navigating them through the healthcare system to ensure that they continue to receive care and support beyond hospital walls.
In the bustling corridors of SGH, the role of a Patient Navigator (PN) is pivotal yet often goes unnoticed. Introduced in 2014, the PNs streamline care coordination, discharge planning, and help patients transit from hospital to community. Mas Rizalynda is among the 23 dedicated PNs in SGH.

When asked what it is that she does, Nurse Clinician Mas Rizalynda says, "I help patients and their families navigate the healthcare system, especially during their most difficult times.”
"I recall assisting a female patient who was ready for discharge after treatment. She had an estranged relationship with her only next of kin, her sister, and was previously neglected at home. The patient was under Adult Protection Service (APS) care. Through discussion with the patient, the care team planned to transfer her to a Transitional Care Facility while waiting for a place in a nursing home to continue her care and full recovery. Despite the sister's insistence on taking her home and multiple complaints to our Office of Patient Experience, I was glad to coordinate community support for the patient and ensured her safety after discharge," explains Mas, a seasoned nurse of 26 years with experience in the Haematology and Medical Oncology Ward.

Mas’ daily responsibilities as a Patient Navigator include assessing patients' care needs by evaluating their medical conditions, background, and ability to manage daily
activities. She coordinates tailored discharge plans, ensuring the patient has the necessary medical equipment at home and community services are in place, and facilitates safe and timely transitions from hospital to home. Collaborating closely with doctors, nurses, and allied health professionals, she serves as a vital liaison and resource, bridging communication between patients, healthcare teams, and community services.
Overcoming challenges
Mas’ journey is not without hurdles. Balancing professional responsibilities with personal roles, such as being a mother and pursuing academic ventures, can be daunting. "My challenge has always been holding different social roles - mainly being a mother and a full-time working professional," she admits.
To overcome these challenges, she recommends reviewing career goals and life priorities periodically, resetting achievements and purposes to stay aligned with both personal and professional aspirations.
Mas was recently awarded the 2024 Nurses Merit Award. "Undoubtedly, my family has been my pillar of strength. Their presence is essential, and most importantly, they have been my top priority all these years," she says, attributing her success to her supportive husband and children.

Mentoring the next generation
With 26 years of nursing experience, Mas loves mentoring younger nurses. Her tip? “Be generous by sharing knowledge freely to enhance both your skills and that of others, be inclusive by imparting wisdom to anyone who seeks it without discrimination, and be patient by understanding different learning curves and providing supportive guidance.”
She also has this nugget for aspiring nurses, “Embrace challenges boldly and care for yourself holistically—physically, mentally, emotionally, and spiritually.”
