"There are palliative patients under our care who wish to be discharged so they can spend their final days at home. My team and I will provide support for the family members of these patients by training them to manage their loved ones’ symptoms, coordinating the loan of medical equipment and advising them on what to do when their loved ones eventually pass on.
Through the provision of this end-of-life support, we hope to help allay the anxiety of family members and fulfil the last wishes of these patients."
